How do I discover out who my GP is???

I'm filling in a medical form for a course, and information technology wants to know, among other things, the details of my GP.

How do I find this out?

I oasis't been to encounter a doc for about eight years. I couldn't even name a doc working at my local surgery.

  • I simply normally write down the surgery name - I never normally write the doctors proper noun cause one. I don't know who he is and ii. I've never ever been to come across my actual doctor.

    You could call the surgery though, the receptionist should be able to tell you

  • Are yous registered with a clinic/GP?

    If non then find your nearest one and sign upwardly. When you sign upwards they will meet if yous are signed up elsewhere.

    If you are and then but call them for the details of your GP.

  • Phone call the surgery where your records are held. You will accept a doc assigned to you. They tin can tell you.

  • I only put the surgery name commonly, I never see the same doctor anyway.

  • I only always put down the name and address of the medical practise - I rarely go to the GP and never see the aforementioned person twice. If for any reason they wanted a medical report, they would have to get your signed consent beforehand anyway.

    No one really has a personal gp anymore! just put the exercise name :)
